Coaches & Team Checklists

A Team Checklist and Action Plan is provided as a guide to developing the infrastructure to
implement PBS on a local level. The Team Checklists are a self-assessment report and are
reviewed regularly by the PBS Leadership Team to identify areas of strengths and areas that
require more technical assistance. An implementation rate of 80% on the Team Checklist
(when corroborated with the SET report) over three semesters indicates the likelihood of sustaining
the initiative.

The Team Checklist is reviewed on a regular basis by the team. Team members reach consensus
on the status of implementation (achieved, in progress, not started) for each of the 17 questions.
After deciding the status of the 17 questions, always prepare an action plan to focus work of the
leadership team. Coaches would input the status of each of the 17 questions on
www.pbssurveys.org no less than 4 times a year (2 weeks before each training or technical
assistance meeting and at the end of the school year). Contact the Alliance office for your password.

Coaches will be prompted by the Alliance office to submit their Team Implementation Checklist data approximately two to three weeks prior to scheduled training.
