Highlights
(a) – Activities: Intensive case management, connecting students/families to community resources,
mental health services, academic support, after-school and summer enrichment programming.

(b) – Outcomes:
For 2003-04:
92% of individuals receiving case management services made progress on their individual goals;
85% of pregnant/parent teens stayed in school;
94% of referrals resulted in a connection with a community resource;
97% of parent survey respondents reported that services received were very helpful or helpful.

New performance measures for 2004-05
:
85% of individuals receiving case management services will make progress on their goals;
75% of elementary and middle school students receiving case management services will read on grade level;
75% of high school students receiving case management services will advance to the next grade level by earning five or more credits;
40% of students receiving case management services will attend school at least 168 days/year;
60% of English Language Learners who receive case management services will improve their Language Assessment Scales score;
75% of pregnant or parenting teens receiving case management services will stay in school;
75% of students receiving one to one mentoring will have a reduction in office referrals;
80% of parent survey respondents will report that services received were very helpful or helpful,
90% of the babies born to mothers receiving case management services will be born at or above normal birth weight;
75% of children receiving case management services will be properly immunized;
70% of children receiving case management services will be insured;
75% of families with children under six will improve parenting skills

Demographics
The SUCCESS Program employs 42 case managers in 31 schools with students identified as high
need within the Des Moines Independent Public School District. For the program year 2003-2204, the
SUCCESS Program served 3,089 individuals. Of those, 2,524 were children/youth under the age of 21,
and 565 were adults. Eighty-five percent of individuals served were of low socioeconomic status.
Fifty-two percent were of minority status. Thirty-six percent were male and sixty-four percent were female.
